Accepted Insurances

Remember: As a patient, when it comes to your physical therapy, you can choose where you receive treatment. You are not obligated to visit the facility recommended by your physician. Accepted insurances include:

If your HMO insurance does not consider PT 360 Physical Therapy as an "in-network" clinic, it is more than likely that they have determined that there is a sufficient number of out-patient clinics in this area, and have denied our application to be in-network.  We continue, on a yearly basis, to apply for in-network acceptance by Blue Care Network and Priority Health.  If you would like PT 360 Physical Therapy to be considered for in-network affiliation, please contact your HMO provider.  If you have PPO or POS insurance that we are not in-network with, you may still have a portion of your therapy paid by insurance.  PT 360 Physical Therapy does provide liberal payment plans, as our number one goal is to relieve pain and break the cycle of pain - not break the bank account!
